Pet euthanasia and cremation
The consent form
You do not need to print this, or fill it in. It is here so you can read it before the day, in your own time. Your vet goes through it with you in the clinic, and nothing happens until you have signed.
Back to pet euthanasia and cremationWhat this form is for
When you have decided to say goodbye, this is the one form you sign with us.
It gives us your permission to go ahead, and it writes down exactly what you want to happen afterwards: the cremation you have chosen, whether their ashes come home to you, and the keepsakes you would like.
It is there for you. On a day when it is hard to think, your wishes are written down, so nothing is left to memory and nothing is misunderstood.
It is there for us too. It is our clear record that we have your permission, and that we know exactly what you want for them.
The form
This is the form as you will see it on paper. Nothing on this page can be filled in or sent.
1You and your pet
2Your permission tick each that applies
- I am the owner of this pet.
- I am acting for the owner, with their permission.
- I confirm that I am 18 or over.
I give Paws & Co, and its vets and team, full permission to put my pet to sleep humanely, and to care for their body and arrange their cremation as I have chosen below.
I release Paws & Co, its vets and its team from all liability for the euthanasia, and for caring for my pet's body, as I have set out in this form.
3Afterwards tick one
- Communal cremation. Your pet is cremated together with other pets. Their ashes are not returned.
- Individual cremation. Your pet is cremated in their own separate, marked compartment, and their ashes come home to you.
- Private cremation. Your pet is cremated on their own, and their ashes come home to you.
4Cremation certificate and keepsakes
A cremation certificate tick one
You need a cremation certificate if you ever want to take their ashes to another country.
- Yes, I would like a cremation certificate.
- No, thank you.
Keepsakes tick any
- A paw impression
- A memorial card, with their photo, their date of birth and the date they passed. You can share your favourite photo with us.
There is no charge for either. They are part of the service.
5Your signature
I have read this form and I understand it. The details I have given are true, as far as I know. The costs have been explained to me. I understand that my wishes may be carried out as soon as I sign.
